Intercostal Nerve Blocks
Pain Treatment Procedure
Intercostal Nerve Blocks
Intercostal nerve blocks (ICNB) are a diagnostic and therapeutic injection used to relieve chest and rib pain. During the minimally invasive procedure, medicine is injected directly into the intercostal nerves that run between your ribs for targeted relief of chronic chest wall, rib or upper abdominal pain. From breathing deeply to sleeping comfortably, this type of pain and discomfort can dramatically affect your daily life.
By calming these nerves, ICNBs can significantly reduce pain and inflammation, thereby improving your quality of life.[1]
How Intercostal Blockades Work
During the procedure, a pain management specialist carefully injects a combination of local anesthetic and sometimes steroids near the affected intercostal nerves.[1] These nerves sit just beneath the ribs, so the injection is typically performed under image guidance (such as ultrasound or fluoroscopy) for precision and safety.[2]
The anesthetic helps numb the nerve and block pain signals to the brain, while the steroid reduces inflammation that may be irritating the nerve. Most patients experience relief within minutes to hours after the injection, and the effects can last anywhere from several days to several months, depending on the condition and individual response to treatment.[3]
Conditions Treated with ICNBs
Intercostal nerve blocks can be used for different reasons. One is to give you relief from either sudden (acute) or long-lasting (chronic) pain. By calming the nerves and reducing inflammation, the injection may also give your body a chance to heal. This type of nerve block may help with pain caused by:
- Anesthesia or post-surgical pain from thoracic, breast, chest or upper abdominal surgery (e.g., after lung or rib surgery) [1]
- Shingles or postherpetic neuralgia [1]
- Rib fractures or injury-related pain [1]
- Chronic chest wall pain [4]
- Chronic upper or mid-back pain (thoracic spine) [4]
- Intercostal neuralgia, a nerve disorder affecting the space between the ribs [5]
This treatment is often recommended when other therapies haven’t provided adequate relief or when inflammation and nerve irritation are the root cause of the pain.
Intercostal and paravertebral blocks can treat some of the same conditions, but they target different nerves with the medication. [6] Your pain specialist will help decide which option is best for your unique needs.
Benefits of Intercostal Blockades
If you’ve been living with chest, rib, or upper abdominal pain that won’t go away, an intercostal nerve block could offer real relief. These injections are more than just a quick fix; they can be a helpful tool in a bigger, whole-body pain management plan. Here are some key benefits:
- Targeted Pain Control: Unlike oral medications that affect your whole body, ICNBs are delivered directly to the nerves causing pain, providing focused relief where you need it most. [2]
- Fast-Acting Relief: Many people experience pain relief within minutes to hours after the injection, allowing you to breathe, move, and rest more comfortably. [1]
- Reduced Reliance on Medication: Nerve blocks can lower your reliance on opioids or other long-term medications, helping you avoid unwanted side effects or dependency. [4]
- Minimally Invasive: This outpatient procedure is quick and doesn’t require surgery, stitches, or a long recovery. Most patients are in and out the same day.
- Improved Mobility and Function: By reducing pain and inflammation, ICNBs can help you move more freely and get back to physical therapy or your daily routine sooner.
- Diagnostic Insight: These injections can also help confirm whether your intercostal nerve is the source of your pain, allowing your provider to tailor the right treatment plan. [2]
- Whole-Body Benefits: Unlike medications that affect your entire system, intercostal nerve blocks work at the source without clouding your thinking, upsetting your stomach, or affecting your energy.
When used as part of a multidisciplinary approach, such as combining intercostal nerve blocks with physical therapy, lifestyle changes, and other treatments, they can be a powerful tool for long-term pain control and improved quality of life. [7]
Consult an Intercostal Nerve Block Specialist for Pain Management
Are you a good candidate for an ICNB? This treatment may be a good fit if your chest or rib pain hasn’t improved with medications, physical therapy, or other conservative treatments. It’s also commonly recommended when imaging or physical exam results suggest that nerve irritation or inflammation is the source of your pain. If you’re looking for a non-surgical option that offers real, lasting relief, an intercostal nerve block could be worth considering.[8]
Intercostal nerve blocks often work best as part of a whole-body approach to pain management. That means combining them with physical therapy, breathing exercises, lifestyle adjustments, and other treatments that support healing from every angle.
Chronic pain doesn’t have to put limitations on your life. Talk to your pain management physician to find out if an intercostal nerve block might be the breakthrough your body needs.
